General Enrollment

Please review the information below to begin NEW-TO-THE-DISTRICT enrollment and registration. You will need to schedule a time with the school office to finish the registration process. If you have any questions, please contact the board office at (440) 988-4406. Additional forms and information to complete the process are also below!

NOTE: This information and online form is NOT to be used for Returning Student Registration. The yearly returning student registration process needs to be completed in you PowerSchool Parent Portal account.

Registration Process

1. Registration Requirements (This is a checklist to help you collect the required documents for registration.)

2. Student Registration Form (Complete this paper version of the required Student Registration Form OR complete the online version below.)

3. Release of Records Request Form

4. Notarized Residency Affidavit

5. Directory Information Release

6. Pupil/School Transfer Slip

7. Call the school office to schedule an appointment to finalize the registration process!

Bring each of these original documents:
*Certified Birth Certificate*Immunization Record
*Current Custody Papers*Guardianship Papers (if applicable)
*Parent Photo ID

Bring two of the following as Proof of Residency that shows current address:
*House Payment/Deed*Lease/Rental/Purchase Agreement
*Pay Stub or Cable Bill*Home/Auto Insurance Statement
*Food Stamp Verification*City/County Property Tax Statement
*Notarized Residency Affidavit
*Voter Registration Card

Note: Utility bills and credit card statements are not a board-approved proof of residency.
